A sharp foreign body that perforates the bowel may migrate distally, leaving the surgeon with a perforation but no object to retrieve. We report a fish bone that perforated the mid-ileum and migrated to the descending colon, eluding laparoscopy and intraoperative enteroscopy. A plain intraoperative radiograph relocalized the bone and enabled targeted endoscopic retrieval. The computed tomography, laparoscopy, X-ray, endoscopy framework summarizes this experience as a case-derived diagnostic cascade in which each step’s findings dictate the next; in this patient it reduced futile bowel manipulation and preserved minimally invasive management. |
