Core Tip We developed and validated a prediction model incorporating an objective marker of nutritional reserve to estimate 12-month risk of overt hepatic encephalopathy (OHE) after initial transjugular intrahepatic portosystemic shunt (TIPS) placement in patients without prior OHE. The simplified age–bilirubin–creatinine–psoas muscle thickness (APTC) score (range 5-14 points) showed higher discrimination than the Model for End-Stage Liver Disease, Child-Pugh, Freiburg Index of Post-TIPS Survival, and, in an exploratory comparison, Modified TIPS Score. Using routine preprocedural imaging and laboratory data, APTC may support risk communication and closer monitoring in this primary-prevention population. However, it remains unvalidated in patients with previous OHE and should not, by itself, trigger prophylaxis.
