The neurotrophic hypothesis positions brain-derived neurotrophic factor (BDNF) centrally in depression pathogenesis, but upstream regulators of the precursor brain-derived neurotrophic factor (proBDNF)/mature brain-derived neurotrophic factor (mBDNF) balance remain unclear. Emerging evidence suggests astrocytic aquaporin-4 (AQP-4) modulates BDNF isoform secretion through calcium signaling and glutamate homeostasis. AQP-4 loss, mainly in cellular, animal, and postmortem studies, has been linked to increased proBDNF/mBDNF, pro-apoptotic signaling, and impaired synaptic plasticity, implying an AQP-4–BDNF axis. However, most evidence is preclinical, with limited causal data in human major depressive disorder. Thus, the AQP-4-BDNF axis is a plausible, hypothesis-generating candidate requiring further investigation, not an established therapeutic target. |
