Core Tip This case describes laparoscopic subtotal colectomy with transvaginal natural orifice specimen extraction in a patient with situs inversus totalis and refractory slow-transit constipation. The main technical lessons include careful preoperative recognition of mirror-image anatomy, mirror-adapted trocar placement, dynamic surgeon repositioning, and meticulous identification of reversed vascular and organ anatomy. Because this is a single case and complete anorectal physiology testing was not performed, the procedure should be interpreted as technically feasible only in carefully selected patients.
