Core Tip Perineural invasion is an established prognostic marker, but its independent predictive value has not been demonstrated. By separating colon from rectal cancer and considering stage and neoadjuvant-treatment status, this review classifies the evidence as established, promising but unvalidated, or investigational. Clinically, perineural invasion is best interpreted together with TNM stage, lymphovascular invasion, tumor budding, molecular status, and treatment response. Depth mapping, radiomics, artificial intelligence, and biomarker approaches should remain within validation studies.
