Core Tip We describe a previously underrecognized condition known as oxaliplatin-associated pseudosepsis syndrome that occurs during hepatic arterial infusion chemotherapy. It is characterized by high-grade fever, marked elevations in procalcitonin and interleukin-6 levels, transient hypotension, and occasional multiorgan dysfunction despite consistently negative blood cultures. The temporal pattern, universal recurrence upon rechallenge, and favorable response to supportive care strongly support a drug-associated inflammatory response rather than infection. Recognition of this entity is critical to avoid misdiagnosis, unnecessary antibiotic exposure, and treatment delays. In selected patients, substitution with alternative platinum agents may be considered, although further evidence is required.
