Core Tip	Organ preservation requires appropriate selection, response verification, surveillance, and timely salvage. Endoscopic treatment of low-risk superficial tumors and rectal watch-and-wait in experienced programs have the most mature evidence. Esophageal surveillance remains protocol-dependent despite meeting a two-year non-inferiority endpoint. Sentinel-node-guided gastric preservation has not established oncologic non-inferiority. Pancreatic parenchyma-sparing strategies mainly concern benign, premalignant, or low-grade lesions and cannot be extrapolated to pancreatic ductal adenocarcinoma. Retained anatomy should be evaluated alongside patient-reported function and cancer control.
