Core Tip This study provides a comprehensive single-centre South Asian longitudinal analysis of pregnancy after kidney transplantation with extended graft-survival follow-up. Despite high rates of hypertension, preterm birth, and graft dysfunction, no maternal or neonatal mortality occurred, and overall graft function remained stable. Hence, successful pregnancy may be achieved with proper multidisciplinary care and careful surveillance. Baseline serum creatinine and pre-existing hypertension predicted adverse pregnancy outcomes, while higher baseline creatinine tracked with inferior long-term death-censored graft survival. These findings emphasise pre-conception optimisation, pregnancy-compatible immunosuppression, and timely counselling to improve both obstetric and long-term transplant outcomes.
