| Core Tip |
Iatrogenic bile duct injury is a severe complication of laparoscopic cholecystectomy, often caused by unrecognized anatomical variations. Pre-operative magnetic resonance cholangiopancreatography can easily overlook uncommon variants, such as a ring-like communicating accessory bile duct (CABD), due to spatial resolution limitations or overlapping fluid signals. This case demonstrates that intraoperative low-dose indocyanine green fluorescent cholangiography offers helpful visual guidance, assisting surgeons in recognizing complex ring-like CABDs and associated impacted lithiasis to achieve the critical view of safety. |
