Core Tip	This case report demonstrates that a single-incision plus one-port approach was successfully performed for concurrent splenectomy, pericardial devascularization, and hepatectomy in a selected cirrhotic patient with portal hypertension and a liver tumor in segments VI/VII. Splenic pedicle suspension, bilateral extracorporeal traction, and microwave pre-ablation were used to address the technical challenges of pure single-port access. The procedure was completed without conversion, with favorable short-term outcomes.
