| Core Tip |
D2 plus lymphadenectomy in gastric cancer should be tailored to tumor biology and treatment response. Pathological response to neoadjuvant therapy is the strongest prognostic factor, whereas the emerging molecular characterization (microsatellite instable, programmed cell death ligand 1, epithelial-mesenchymal transition, or human epidermal growth factor receptor 2 status) predicts nodal spread pattern and tumors which likely benefit from extended nodal dissection. In selected oligometastatic patients, para-aortic lymphadenectomy can be integrated into a conversion surgery strategy to achieve complete radicality. Molecular profiling, therefore, represents a promising tool to refine treatment course and surgical indications for lymphadenectomy beyond tumor node metastasis staging alone. |
