Artificial intelligence (AI) is rapidly transforming gastrointestinal (GI) endoscopy by enhancing lesion detection, characterization, procedural quality, and clinical decision-making. This review summarizes current evidence across lower and upper GI endoscopy, capsule endoscopy, endoscopic ultrasound, and endoscopic retrograde cholangiopancreatography. The strongest clinical evidence currently supports computer-aided detection during colonoscopy, particularly for improving adenoma detection, whereas other applications remain at varying stages of validation. Emerging multimodal, generative, and robotic AI technologies may further advance precision endoscopy. However, robust external validation, algorithm transparency, interoperability, regulatory oversight, and evaluation of clinically meaningful outcomes remain essential before widespread implementation in routine practice. |
