Core Tip A plasma variant is not automatically tumor-derived. This review distinguishes direct per-call provenance evidence from proxy, feature-development, and adjacent detection evidence. Computational models show promise, but gastrointestinal validation and clinical utility remain limited. Figures 1 and 2, the five-stage workflow, and the worked examples are author-proposed, unvalidated conceptual tools, not clinical guidelines. They retain mixed and indeterminate states and use artificial intelligence to prioritize confirmation while preserving matched leukocyte, tumor, and accredited germline testing for consequential decisions.
