Core Tip This review integrates immunosenescence and compartment-specific microbial ecology within the context of gastrointestinal and hepatobiliary infection-associated cancers. We distinguish immunosenescence from T-cell exhaustion and cellular senescence, and compare evidence across Helicobacter pylori-associated gastric cancer, hepatitis B/C virus-associated hepatocellular carcinoma, and liver-fluke-associated cholangiocarcinoma, with colorectal cancer as a mechanistic comparator. Particular attention is given to anatomical compartment, evidence directness, disease stage, and reversibility after pathogen control. Current evidence supports several mechanistic links, but a complete microbiota–immune aging–cancer causal sequence in humans remains unproven. These considerations support a pathogen-first approach to clinical translation and highlight priorities for longitudinal, compartment-matched studies.
