Core Tip Bacterial extracellular vesicles (BEVs) provide a framework for understanding how microbial signals influence mesenchymal stem/stromal cell (MSC)-mediated bone repair. Available evidence comprises direct effects on MSCs/bone marrow-derived MSCs, supportive evidence from other osteogenic-lineage cells, and indirect effects mediated by macrophages, osteoclasts, and endothelial cells. These actions depend on vesicle source, cargo, dose, target-cell state, and disease context. Increases in bone mass or reductions in bone resorption should not be equated with demonstrated fracture or bone-defect regeneration. Defining these evidence boundaries and developing controllable engineered BEVs are essential for translation.
