Core Tip Whether real-time artificial intelligence (AI) improves neoplasm detection during upper gastrointestinal endoscopy remains debated following a large, centrally adjudicated gastric trial that found no significant difference. This systematic review and meta-analysis of 14 randomized trials (90828 participants) found that AI increased per-patient neoplasm detection, with similar estimates for the esophagus and stomach, and no evidence that effects varied by system functionality. The esophageal evidence was homogeneous and of moderate certainty, whereas the gastric evidence remained heterogeneous and of low certainty. AI is a promising adjunct whose clinical benefit requires confirmation.
