Key Words Liver transplantation; Waiting list; Model for end-stage liver disease; Portal vein thrombosis; Hepatocellular carcinoma; Transjugular intrahepatic portosystemic shunt; Frailty; Sarcopenia; Prehabilitation
Core Tip The liver transplant waiting period is an active therapeutic window rather than an administrative interval. Allocation scores quantify short-term medical urgency but incompletely capture portal hypertensive complications, tumor progression, frailty, sarcopenia, and nutritional decline. This review integrates dynamic risk assessment with waitlist-specific management of portal vein thrombosis, hepatocellular carcinoma, and transjugular intrahepatic portosystemic shunts, and examines prehabilitation as a potentially modifiable determinant of outcome. A risk-adapted, multidisciplinary strategy may help preserve transplantability, although prospective studies using waitlist-specific endpoints remain necessary.
