Core Tip Herein, we present a unique report of a woman with a viable transplanted uterus graft who was suffering from concurrent Clostridium difficile colitis and acute appendicitis. The differential diagnosis in an immunosuppressed patient may be complex, and a high index of suspicion of an atypical course of acute abdomen should be maintained in immunosuppressed patients. Sometimes, more than one synchronous pathological condition may be present. This case highlights the ultimate importance of focusing on the achievement of successful embryo transfer in the UTx recipients as soon as possible since they are vulnerable to a multitude of possible complications.
