Core Tip This study compared short course radiotherapy (5 Gy in 5 fractions) with conventional fractionated radiation treatment (1.8 to 2.0 Gy in 25 to 28 fractions) plus chemotherapy in 100 patients with stage IIIB or more severe rectal cancer to verify whether short course preoperative radiotherapy had an advantage in diminishing complications, reducing risk of recurrence and improving survival. The findings revealed that there was no significant difference in the incidence of complications between short and long course radiotherapy plus chemotherapy. However, short course radiotherapy improves 1-year survival rate and reduces recurrence rate and distant metastasis rate in comparison with conventional preoperative radiotherapy.
