Core Tip D2 radical operation has been the standard treatment of gastric cancer. One in ten advanced gastric cancer patients had lymph node metastasis at station 8p, but D2 Lymphadenectomy remains technically difficult. The new superior pancreatic lymphadenectomy with portal vein priority via posterior common hepatic artery approach had achieved safe removal of 8p lymph nodes.
