Core Tip This is the first study to compare the difference in the failure patterns for pT3-4N0-3M0 esophageal squamous cell carcinoma (ESCC) patients with or without post-operative radiotherapy (PORT) after esophagectomy. The result showed that PORT could improve disease-free survival and local control of patients with stage pT3-4N0-3M0 ESCC. However, distant metastasis was the main failure pattern after receiving PORT. Further studies need to be conducted to control hematogenous metastasis in the future.
