Core Tip In this retrospective study involving 498 consecutive patients who underwent high-resolution esophageal manometry, we found that impaired relaxation alone was the most common upper esophageal sphincter (UES) abnormality. The incidence rate of type Ⅱ achalasia was significantly higher in subjects with abnormal UES compared with normal UES. Subjects with type Ⅱ achalasia and abnormal UES presented a significantly poorer treatment response than those with normal UES.
