Core Tip We present a case of medial malleolar fracture, and the associated complete syndesmosis injury with interosseous membrane rupture was overlooked, which teaches us to pay more attention to thorough physical examination, exact reading of auxiliary imaging and intraoperative stress fluoroscopy to avoid misdiagnosis. Furthermore, this is the first report describing this pattern of injury as a Maisonneuve-equivalent injury and further classifying Maisonneuve injury into three types.
