Core Tip This prospective observational study highlights a simple, reproducible, and radiation-free method to confirm central venous catheter tip position at the bedside by combining the saline flush test with a structured four-point ultrasound evaluation. Unlike traditional chest radiography, which is delayed, resource-intensive, and exposes patients to ionizing radiation, this ultrasound protocol provides instant functional and anatomical confirmation of catheter tip placement. By offering rapid detection of malposition, minimizing complications, and streamlining clinical workflow, this approach can be readily adopted as a first-line tool in perioperative and critical care settings, especially where time-sensitive decision-making is crucial. Its simplicity also makes it feasible for clinicians with basic ultrasound training, paving the way for broader integration of point-of-care ultrasound into routine vascular access practices.
