Laparoscopic redo anastomosis remains rarely reported, and practical guidance for real-world decision-making is limited. This case report demonstrates a stepwise approach to laparoscopic redo ileocolic anastomosis, emphasizing early assessment of peritonitis, careful evaluation of retroperitoneal adhesions near the duodenum and pancreas, and key technical strategies to avoid major complications. This report provides practical insights for surgeons considering a minimally invasive approach in carefully selected patients.
