Core Tip	A 42-year-old male patient with cubital tunnel syndrome caused by an unusual accessory ossicle, os subepicondylare mediale, had complaints of pain in the elbow, tingling in his fingers and a weak grip. Ulnar nerve compression was confirmed by imaging. Symptoms and functional recovery were improved with surgical decompression and ossicle removal, demonstrating the importance of considering ossicles in ulnar neuropathy and the effectiveness of surgical treatment.
