Core Tip Post-transplant hepatocellular carcinomarecurrence occurs in 8%-20% of recipients, mainly within 2 years. Key drivers include immunosuppression, tumor biology, and ischemia-reperfusion injury. Risk factors are beyond Milan criteria, microvascular invasion, and high alpha-fetoprotein. Surveillance uses imaging and alpha-fetoprotein, with circulating cell-free DNA-minimal residual diseaseenabling earlier detection. Oligorecurrence benefits from resection or ablation. Advanced disease is treated with tyrosine kinase inhibitors, while immune checkpoint inhibitors risk rejection. Adjuvant lenvatinib improves recurrence-free survival in high-risk patients. artificial intelligence-based models enhance prediction, but evidence remains largely retrospective, requiring prospective trials.
