Core Tip	This report presents an exceptionally rare case of coexisting esophageal verrucous carcinoma (EVC) in situ and gastric intramucosal poorly differentiated adenocarcinoma. It highlights the major diagnostic challenge of EVC, as superficial biopsies are frequently non-diagnostic and mimic benign conditions. The case underscores the critical role of endoscopic submucosal dissection (ESD), which served not only as a curative treatment but also as the definitive diagnostic procedure. By providing a full-thickness specimen, ESD ultimately confirmed the diagnosis and enabled a favorable prognosis through complete resection.
