Core Tip This study demonstrates that combining transjugular intrahepatic portosystemic shunt with 125I radioactive seed strand implantation significantly improves survival and reduces stent restenosis in patients with portal hypertension and portal vein tumor thrombus. This combined approach addresses both the hemodynamic consequences of portal hypertension and the oncologic burden of tumor thrombus, representing a paradigm shift toward comprehensive management of this challenging condition.
