Core Tip Older patients after cardiothoracic surgery are at high risk of behavioral and psychological symptoms of dementia (BPSD), while effective non-pharmacological treatments remain limited. This retrospective cohort study showed that intermittent theta burst stimulation (iTBS) targeting the left dorsolateral prefrontal cortex was associated with reduced BPSD severity, improved cognitive function, and better daily living performance, with no statistically significant difference in adverse event rates between the two groups. Exploratory findings further suggested that enhanced gamma-band brain connectivity and reduced inflammatory cytokines may be involved in the therapeutic effects of iTBS. These results indicate that iTBS may be a safe and promising treatment option for postoperative BPSD in older adults.
