Core Tip	This study retrospectively analyzed the data of our center and conducted a meta-analysis with 20 studies evaluating the potential risk of major low anterior resection syndrome (LARS) in patients with rectal cancer after sphincter-preserving surgery. Seven potential risk factors were found to be related to long-term major LARS after surgery, including shorter distance from anal verge, neoadjuvant therapy, adjuvant radiotherapy, adjuvant chemotherapy, protective stoma, intersphincteric resection, and anastomotic leakage. And the incidence of long-term major LARS after radical resection of rectal cancer was 35% (95%CI: 29%-41%, I² = 96%).
