Core Tip This is a meta-analysis of 10 randomized controlled trials from around the world comparing laparoscopic vs open hepatectomy for hepatocellular carcinoma (HCC) in 587 patients with indications. The cumulative incidence of complications at 1 month after surgery was significantly lower (odds ratio = 0.52, P < 0.01) in the laparoscopic hepatectomy (LH) group, confirming clear short-term benefits in terms of postoperative morbidity with this approach. While patients with LH suffered lower 5-year overall survival than their counterparts after adjustment (hazard ratios = 1.34, P < 0.01) and borderline inferior progression-free survival (P = 0.04). Three-month rates of recurrence were not significantly different across approaches. These findings exemplify the clinical dilemma: LH is associated with short-term safety benefits, which should be carefully balanced against long-term oncological compromise when choosing surgical approach for HCC in our patient cohort.
