Core Tip In 427 patients with locally advanced gastric cancer treated with neoadjuvant chemotherapy and radical gastrectomy, tumor regression grade (TRG) independently predicted recurrence-free survival and was associated with recurrence timing and pattern. Peritoneal recurrence was most common and had the poorest outcomes. TRG 2-3 was linked to earlier locoregional/peritoneal relapse, whereas distant recurrence was relatively more frequent in TRG 0-1, supporting postoperative risk stratification.
