Core Tip Autoimmune gastritis (AIG) is a progressive disease with increasing clinical consequences as gastric atrophy advances, yet no validated endoscopic staging system currently exists. We developed and validated the AIG atrophic staging (AIG-AS) classification based on the extent of remnant oxyntic mucosa. AIG-AS closely reflected both biological disease progression and clinical impact, showing significant associations with parietal cell antibody titers, gastrin levels, pepsinogen (PG) 1 levels, the PG 1/2 ratio, and anemia risk. These findings support the clinical validity of AIG-AS as the first endoscopic staging system for AIG to be validated against biological and clinical disease progression.
