Core Tip In this single-center retrospective cohort, 78 of 195 septic shock survivors (40.0%) met an operational high-stress threshold of Impact of Event Scale-Revised (IES-R) ≥ 33 at intensive care unit (ICU) discharge or day 7. High-stress status was associated with greater illness severity, more intensive ICU treatment, biomarker abnormalities, and worse unadjusted 90-day outcomes. Because several variables in the multivariable model were measured during or after the index assessment window, the model is best interpreted as an exploratory retrospective association/classification analysis rather than a validated early prediction tool. Baseline IES-R was removed from receiver operating characteristic comparisons because IES-R defined the high-stress outcome.
