Core Tip There remains limited evidence to guide the optimal first-line systemic therapy for unresectable or metastatic combined hepatocellular-cholangiocarcinoma (cHCC-CCA), a rare and aggressive liver cancer. This multicenter retrospective cohort study evaluated the efficacy and safety of programmed cell death protein 1 inhibitor-based combination therapy in 31 patients with unresectable or metastatic cHCC-CCA. The regimen demonstrated a median overall survival of 15.8 months and a manageable safety profile. Furthermore, elevated baseline levels of serum carcinoembryonic antigen and carbohydrate antigen 19-9 may serve as independent prognostic biomarkers for survival. This evidence supports the potential role of this combination as a treatment option for this challenging disease.
