Core Tip	This interim randomized controlled trial evaluates subanesthetic esketamine as an adjunct to electroconvulsive therapy in adolescents with major depressive disorder, finding it is associated with faster reductions in suicidal ideation and greater improvements in depressive and psychopathological symptoms, without significant short-term global cognitive decline. These preliminary findings show a reduction in suicidal ideation that may reflect concurrent depressive symptom improvement. Further follow-up is needed to assess durability, long-term cognition, and overall safety.
