Core Tip This study evaluated the relationship between serum and intrahepatic hepatitis B virus (HBV) markers across all immunological phases of chronic hepatitis B in a large cohort of treatment-naïve patients. Serum hepatitis B core-related antigen and HBV RNA better reflected intrahepatic pregenomic RNA and covalently closed circular DNA (cccDNA) transcriptional activity than hepatitis B surface antigen in a phase-dependent manner. Notably, no serum marker reliably reflected cccDNA transcriptional activity in the Hepatitis B e antigen-negative chronic HBV infection phase, highlighting its virologic heterogeneity. These findings support the phase-specific application of serum biomarkers for the noninvasive assessment and monitoring of intrahepatic HBV activity, with potential implications for therapeutic decision-making.
