Core Tip	This review included 17 reports. Outcome-level extraction showed that studies contributed to different outcomes and that intraoperative doses, rescue-only medication, median-only summaries, and postoperative opioid totals could not be interchanged. Randomized evidence suggests earlier first flatus and less postoperative nausea and vomiting, but certainty is very low and the postoperative nausea and vomiting estimate includes no effect after high-risk trials are removed. Other gastrointestinal milestones and hospital stay remain uncertain. Differences in regional techniques, systemic drugs, antiemetics, and postoperative care prevent attribution of effects solely to opioid reduction. A uniform overall-complication risk was not estimable.
