This retrospective cohort study compared kidney graft survival between living-related and spousal living-unrelated donors. Spousal transplantation was associated with greater human leukocyte antigen mismatching and lower unadjusted graft survival; however, donor type was not an independent predictor of graft loss after adjustment. These findings support careful immunological assessment while highlighting spousal living donation as a valuable treatment option in regions where deceased-donor kidney transplantation remains limited. |
