Core Tip Pediococcus acidilactici is an exceedingly rare cause of pyogenic hepatic abscess and may occur in apparently immunocompetent individuals without identifiable predisposing factors. Recognition is clinically important given the organism’s intrinsic vancomycin resistance. Management should emphasize adequate source control and individualized antimicrobial therapy. In this case, percutaneous drainage followed by ceftriaxone-based therapy and oral amoxicillin-clavulanate was associated with complete radiographic resolution.
