| Core Tip |
Pediatric inflammatory bowel disease shows marked heterogeneity in both disease biology and therapeutic response, making biologic selection challenging in routine practice. Although anti-tumor necrosis factor agents, vedolizumab, ustekinumab, and emerging interleukin-23 inhibitors have transformed outcomes, primary non-response and secondary loss of response remain common. This review summarizes current evidence on clinical, biochemical, pharmacokinetic, genetic, transcriptomic, immunologic, and microbiome-based predictors of biologic response in children with inflammatory bowel disease. It highlights therapeutic drug monitoring as the most established tool while emphasizing the limitations of single biomarkers. The review further discusses biologic-specific predictive signatures and the growing role of integrated multi-omics and artificial intelligence approaches. Collectively, these advances support a shift toward precision medicine, aiming to match the right biologic to the right patient at the right time. |
