Core Tip	Interoceptive uncertainty is not poor interoceptive accuracy. It concerns what bodily state is present, how reliable the signal is, what the sensation means, and which action should follow. This review links these questions to measurable parameters, insula-vagal-respiratory mechanisms, and distinct bodily phenotypes across depression, anxiety, obsessive-compulsive disorder, and psychosis. It also separates established clinical components from emerging mechanistic probes and proposes a modular route from multidimensional phenotyping to measurement-informed and adaptive intervention.
