Diffuse large B-cell lymphoma (DLBCL) is a biologically heterogeneous disease requiring precision medicine approaches beyond standard rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone immunochemotherapy. This review synthesizes current evidence on integrating molecular subtyping (cell-of-origin and genetic subtypes), circulating tumor DNA monitoring, and tumor microenvironment characterization into clinical decision-making. We critically evaluate emerging targeted therapies including Bruton tyrosine kinase inhibitors, BCL-2 inhibitors, antibody-drug conjugates, bispecific T-cell engagers, and chimeric antigen receptor T-cell therapy. Prognostic models have evolved from the International Prognostic Index to dynamic, integrative nomograms incorporating molecular biomarkers. Challenges in standardization, cost-effectiveness, and equitable access are discussed alongside future directions for personalized DLBCL management. |
