Core Tip Computer-aided detection raises adenoma detection by about a fifth, but pooled randomized data have not established a benefit for sessile serrated lesions (risk ratio 1.10, 95% confidence interval: 0.93-1.30). Serrated detection was assessed only as an underpowered secondary endpoint, so a worthwhile effect remains possible. Water exchange and longer withdrawal have shown serrated signals in separate studies, but no adequately powered head-to-head trial with a serrated primary endpoint has established comparative effectiveness. Evidence of reduced unassisted vigilance is limited and observational. Serrated-specific trials and monitoring are needed before any effect on interval cancer can be inferred.
