Core Tip	Primary biliary cholangitis (PBC) is increasingly recognized as a spectrum of distinct clinical-immunological forms rather than a single, uniform disease. This review proposes a practical classification of six forms of PBC “classic” antinuclear antibodies (ANA)-positive with cholestasis, AMA-negative/seropositive, completely seronegative, PBC-autoimmune hepatitis (AIH) variant syndrome, AMA-positive PBC without cholestasis, and PBC-primary sclerosing cholangitis variant syndrome-based on autoantibody profile, biochemistry, and histology. A diagnostic algorithm is suggested to help clinicians better recognize these forms of PBC, provide individualized and timely treatment, and enhance surveillance. Of the six forms, three (classic AMA-positive PBC, AMA-negative PBC and PBC-AIH variant syndrome) correspond to entities recognized in international guidance; the remainder are conceptual categories proposed by the authors. The classification and algorithm are hypothesis-generating, are not guideline-endorsed, and require prospective validation before use in practice.
