Core Tip Difficult laparoscopic cholecystectomy is better viewed as an operative problem than as a single clinical entity. The factors associated with difficulty are not the same in symptomatic gallstone disease, acute cholecystitis, after previous biliary intervention, or during interval surgery. Preoperative prediction can help with preparation, surgeon allocation, informed consent, and planning for bailout, but whether dissection should continue is decided by the findings at operation. If the anatomy remains unclear or the critical view of safety cannot be obtained safely, bailout should be regarded as a sound safety decision rather than a technical failure.
