Core Tip	Particularly in high-incidence regions such as India, despite the substantial advances in gallbladder carcinoma research, clinically validated biomarkers for early diagnosis, prognostic stratification and therapeutic guidance remain unavailable. Existing reviews have largely focused on individual molecular aspects without integrating emerging multi-omics technologies into a translational framework. This critical gap is addressed by this review by comprehensively synthesizing current evidence on genomic, transcriptomic, proteomic, exosomal and liquid biopsy biomarkers alongside precision therapeutics. Furthermore, it highlights unmet clinical needs, Indian research priorities and future directions for biomarker validation and precision oncology to facilitate the translation of these molecular discoveries into routine clinical practice.
