Core Tip Circulating microRNAs (miRNAs) have emerged as promising non-invasive candidate biomarkers for the assessment of transplant injury. The aim of this study was to evaluate circulating miRNA profiles as exploratory candidate biomarkers for detecting rejection in heart transplant (HTx) recipients within the first five years post-transplantation. Plasma samples were collected from 20 HTx recipients with antibody-mediated rejection (AMR), acute cellular rejection, cardiac allograft vasculopathy, and no rejection. Circulating miRNA profiles were generated using next-generation sequencing. Following apeglm shrinkage, hsa-miR-193b-5p, hsa-miR-125b-5p, and hsa-miR-5588-5p were significantly upregulated in recipients with AMR. These preliminary findings require validation in larger independent cohorts before any clinical application.
