Key Words Laparoscopic liver resection; Pringle maneuver; Infrahepatic inferior vena cava clamping; Hemihepatic inflow occlusion; Central venous pressure; Blood loss
Core Tip The Pringle maneuver controls hepatic inflow but cannot directly prevent hepatic venous back-bleeding. Infrahepatic inferior vena cava clamping provides complementary venous decompression and, when combined with the Pringle maneuver, reduces blood loss and transfusion in selected hepatectomies. Selective hemihepatic inflow occlusion may reduce ischemic exposure of the future liver remnant, but its superiority when combined with caval clamping has not been established. Vascular control should therefore be tailored to the source of bleeding, tumor anatomy, functional liver reserve, and hemodynamic tolerance rather than intensified routinely.
